When SEO Breaks, GEO Breaks With It

We’re hearing this question more often from healthcare practices, “Do we still need to prioritize SEO if we’re focusing on AI search and generative engine optimization?”
It’s a reasonable question. AI is reshaping how patients discover healthcare providers. ChatGPT, Google’s AI Overviews, and other generative tools are becoming part of the patient journey. The concern is natural: if AI is the future, shouldn’t we shift resources there?
The answer is more nuanced. SEO and GEO aren’t competing strategies. GEO is built on top of SEO. When your SEO fundamentals break, your GEO visibility collapses with it, often before you realize what’s happening.
What Strong SEO Foundations Look Like
SERO, a radiation oncology practice with 30 board-certified physicians across 20 locations in the Charlotte area, came to us with a clear goal, to educate the public and increase patient inquiries. They had good content. What they needed was strategy.
We focused on aligning their content with Google’s E-E-A-T guidelines – expertise, experience, authoritativeness, trustworthiness. For healthcare, this matters deeply. Patients need to trust what they’re reading. Search engines and AI systems need to trust the source.
The results speak for themselves. SERO increased organic website traffic from 12,000 to 21,000 per month. More importantly, new patient inquiries grew from 2,300 to 6,800 per month. They earned featured snippets on Google and outranked national names like WebMD.

The Authority Signal
Greater Charlotte Oral & Facial Surgery faced a different challenge. Their website was getting traffic, but 88% of it came from outside North Carolina. Nearly half of their visitors were landing on pages in languages they didn’t even offer services in. They had volume without relevance.
We reoriented their entire SEO strategy around local optimization. Geo-targeted content. Local keywords. Structural changes that made it clear to search engines exactly who they served and where.
Within six months, traffic from North Carolina went from 10% to 36% of total visitors. They achieved position 1 rankings for their most valuable local keywords such as “Dental implant surgery Charlotte,” “Wisdom tooth removal Charlotte,” and others. Organic leads grew 152% year over year.
This is important for GEO too. When AI systems pull information about local healthcare providers, they’re looking at the same trust signals and authority markers that SEO revealed: consistent local presence, relevant content, strong reviews, clear positioning.
Weak local SEO means weak local visibility in AI answers.

GEO Depends on SEO
Generative AI systems don’t build their own index the way Google does. They don’t have independent crawlers constantly discovering new web pages. Instead, they retrieve information from content that’s already visible, accessible, and trusted within search ecosystems.
This is critical. Generative systems typically pull from pages that rank well in Google. They rely on the same authority signals, trust markers, and structure that make SEO work.
When your SEO fundamentals weaken, several things happen:
- Your content becomes harder to retrieve. If your site has crawlability issues, broken internal links, or poor page structure, generative systems can’t access it reliably.
- Your authority erodes. If your rankings slip or your website loses visibility in search, AI systems notice. They’re using the same authority signals. When you lose Google’s trust, you lose AI’s trust.
- Your competitors gain ground. While you’re focused on GEO tactics, a competitor with solid SEO foundations is accumulating search visibility, reviews, brand mentions, and authority. AI systems will cite them instead of you.
Moving the Needle
The healthcare practices that win in both search and AI aren’t doing anything mystical. They’re following a deliberate order.
Start with technical SEO
Make sure your site is crawlable, fast, mobile-friendly, and structured clearly. If generative systems can’t read your content, they can’t cite it.
Build topical authority
Create comprehensive, expert-level content that answers patient questions thoroughly. Align with E-E-A-T. Get your physicians’ expertise visible. Make it clear to both search engines and AI systems that you’re a trustworthy source.
Earn visibility in search
Strong keyword rankings, featured snippets, local pack positions, and organic traffic all signal to AI systems that you’re worth citing. This isn’t vanity. It’s a prerequisite for GEO.
Then layer in GEO refinements
Make sure your key information is clearly structured for AI to extract. Include your credentials, your locations, your patient testimonials. Get third-party mentions. Make it easy for generative systems to cite you accurately.
